New Patient Forms

New Patients: please complete the "New patient forms" and bring them to your first naturopathic appointment, or send via email at least 1 business day before your appointment.
Payments for your visits can be made via Cash or Cheque (sorry no debit/credit cards). HST: please note that prices are increasing July 1st, 2010 due to the inclusion of HST.

• Adult Forms (11 pages: Fees, Intake, Consent, Review of Systems, Diet Diary). Available in two convenvient formats:
• Paper Version - for those who prefer to complete by hand OR
• Electronic Version - for those who prefer to complete on the computer; please print or email completed forms (if emailing, please send at least 1 business day before your appointment to ensure it is printed in time)
• Child Forms (under 13 years of age)
• Paper Version (10 pages: Fees, Intake, Consent, Review of Systems, Diet Diary)
