PRE-SCREEN COVID-19:
Please complete our emailed COVID-19 pre-screen before your appointment. If you do not pass the screening questions, your appointment will need to be rescheduled to a later time.
WAITING AREA:
Please continue to sanitize your hands upon entering.
PAYMENTS & RECEIPTS
Payment formats recommended: credit card, Visa-debit or e-transfer or direct billing.
All receipts will be emailed.
INTAKE FORMS
There are no paper intake forms in the waiting area. Please fill out intake forms online prior to your appointment. In case this is not possible, please contact our clinic so we can email you a copy for you to print, complete & bring.
CLEANING:
We are continuing with cleaning protocols to ensure the safety of all staff and patients.
WASHROOMS:
Our washrooms are shared with other businesses. We ask you to wash your hands and/or use our sanitizer after every washroom break.
SELF-SCREEN:
Please complete a self-screening at home. If you are experiencing any of the following symptoms, we are requesting you to cancel or reschedule your appointment to a later date.
